Call him. Tell him how you feel. From the time line that you have given it's been awhile since you two have been together. If you really still have feelings for him and he has the same for you there is no reason in hurting him. He wants you back. Just call him up and say, "I just wanted to tell you that I still love you. Do you still love me?" It's the best way to get your point across without hurting his pride and making it look like you are trying to put him down.
